Combined supra/infratentorial approach to tentorial meningiomas.

چکیده
In spite of significant advancements in imaging technology, monitoring, and microsurgical techniques, complete and safe removal of tentorial meningiomas remains a challenge for most neurosurgeons. Classifications of tentorial meningiomas are revised. The combined supra/infratentorial approach to resect tentorial meningioma is discussed. This approach provides a wider exposure of the supra/infratentorial region with less brain retraction. With this approach the occipital lobe and the cerebellum are exposed along the tentorium. Two illustrative cases are presented. The patients were studied with computerized tomography, magnetic resonance and angiography. The anatomy of the transverse sinus and the confluence of the sinus could be appreciated with these studies. The operative technique is described stepwise. Emphasis is placed on pre-operative evaluation and surgical technique, leading to a total surgical removal of the lesion with margins of safety. The goal of surgical treatment of tentorial meningiomas is their complete and safe removal. With this unique approach we sought to confirm that it offers a safe means of resection not only the neoplasm but also the infiltrated dura.

Increased intracranial pressure and tentorial shear strain.
Tentorial herniation is caused by a pressure gradient between supra and infratentorial compartments and the herniated brain is burdened with a force, which is called shear strain at the edge of the tentorium. The purpose of the present study is to clarify the relationships between increased intracranial pressure, the pressure gradient and the tentorial shear strain. Twenty-three monkeys were us...
متن کاملSupra- and Infratentorial Acute Epidural Haematoma: A Case Report
4 Epidural hematoma is seen in 1 to 9 % of all head injuries, and usually occurs unilaterally. Bilateral Epidural hematoma is an uncommon condition and consists of 2-10 % of all acute Epidural hematomas in adults. Acute supraand infratentorial epidural haematoma is infrequent lesion and only a few articles were published in the literature. The presence of infratentorial haematoma which leads to...

Of 38,510 patients admitted to the Neurosurgical Clinic of Bucharest during the 23-year period 1935-57, 2,757 were operated upon for expanding intracranial lesions. These included 201 cases of tuberculoma in patients of ages varying from 3 months to 68 years, the highest incidence being recorded under 30 years of age, particularly in the age group below 20 (Table I). The relative frequency of t...
متن کاملSurgical Treatment for Falcotentorial Meningiomas
Among intracranial meningiomas, falcotentorial meningiomas, occurring at the junction of the falx cerebri and tentorial dural folds, are extremely rare. Because of their deep location, they are surrounded by critical structures, and have been regarded as one of the most challenging lesions for surgical treatment. In this study, we describe our surgical strategy for falcotentorial meningiomas an...
متن کاملInfratentorial benign cystic meningioma mimicking a hemangioblastoma radiologically and a pilocytic astrocytoma intraoperatively: a case report
INTRODUCTION Cystic meningiomas are rare variants of meningiomas; they can pose a radiological diagnostic dilemma. CASE PRESENTATION We present a rare case of a 30-year-old Chinese woman with a histopathological diagnosis of infratentorial cystic meningioma (World Health Organization Grade 1) in which the features in imaging modalities were suggestive of a hemangioblastoma. Intraoperatively, ...
